Provider Requirements

Practice location: All credentialed providers must have a physical office location where patients will be treated. Since credentialing and contracting can take more than 6 months to a year, providers must have a consistent location for at least 6 months at the time of signing up with Alchemy. If you are moving before 6 months, or are unsure what will happen, we recommend that you wait to credential or expect that there will be additional delays and fees.

TIN/SSN: All credentialed providers must use a Tax Identification Number registered with the U.S. Internal Revenue System.

State License: All credentialed providers must have an active, up to date, state-sanctioned license to practice the specialty that they are credentialed under. If you are a student who is waiting on your license to arrive soon, we may be able to begin the credentialing process on a provisional basis. Contact us for further details.

NPI number:  All credentialed providers must have an NPI number on file with NPPES NPI Registry. To apply for an NPI number, visit their website now.

Malpractice Insurance: All providers must have malpractice liability insurance with a minimum limit of $1 Million per occurence and $3 Million aggregate. If you do not yet have malpractice insurance, we suggest you contact:
